I think I found the problem. Version 7.3p-26 "Updated
iptocountry database from a better source." I see
iptocountry2.csv.enc in /usr/local/surgemail. I removed the
old, obsolete IpToCountry.csv file, and SurgeMail does not block
my US VPN IP address now. Not only that, but if I set my VPN to
CZ, I can still login. :-)
I suspect that the old file left over from ancient versions
caused SurgeMail to exhibit wacky behavior: claiming that a US
IP address was from CZ, and that logins are blocked from country
CZ.
Question: g_deny_country blocks email from specified
countries. Where is the setting to block logins from specified
countries? I'd like to understand why SurgeMail thought logins
from CZ were blocked.
Suggestion: update the documentation to refer to
iptocountry2.csv.enc instead of the old IpToCountry.csv and the
IPv6 companion IpToCountry.6R.csv. (e.g. see https://surgemail.com/knowledge-base/allglobal/#g-country-ip)
Had I seen the new file name in the docs, it would have been a
strong hint that stuff had changed.

problem for years, since at least version 7.3o4-4 and probably
before that. It was a minor annoyance until recently.
