It should have both from and rcpt address, however from in data command might be different with from in command but I think if there is some internal checking process between 2 servers it can create X-IsFriend at the Incoming MX server. No. it doesn't know who the destination users friends are if it's a simple gateway. Or do you mean a surgewall configuration? or forwarding?
