I wasn’t able to find a detailed error message in my login-failed.log because it is filled with "Must issue a STARTTLS command first” messages due to the spammers constantly trying to login. Unsuccessfully, fortunately. In my login.log file I see entries only for the three active accounts I have currently.
Your intuition was right: g_con_perip was set to 10, consistent with the behavior I was seeing. I bumped it up to 40.
It all makes sense now.
